Frequently Asked Questions

Who is eligible for coverage?

5 Age 65 to 74 is for renewal only.

6 50% premium loading for occupation class 4.

7 The maximum renewable age for TPD/PPD is age 69, and the coverage will cease on the policy anniversary when Insured Member attained age 70.

8 For unmarried children Aged 19 to 23 years old nearest birthday if still enrolled in or attending Full-time Higher Education and who are not gainfully employed.

What are the documents required for policy inception?

  • a. Allianz EliteChoice Group Application Form,
  • b. Personal Health Declaration Form (if any),
  • c. Member census in Excel format,
  • d. Appointment letter of authorised personnel(s) on Company’s letterhead (signed and stamped by company), and
  • e. Original certified true copy of passport (for non-Malaysian authorised personnel(s)) or Identity Card of authorised personnel(s) (signed and stamped by Human Resource or Director).

Who is required to complete the Personal Health Declaration form?

Only employees who are:

  • • Aged 65 years old and above; or
  • • Applying for Additional Critical Illness (except Plan 4, 5 & 6) are required to complete the Personal Health Declaration Form and subject to underwriting.

What are the premium payment options?

Only Annual mode of payment is allowed.

Is “Cash Before Cover” applicable?

“Cash Before Cover” is applicable for policies with annual premium of less than RM10,000 excluding stamp duty and service tax. “Cash Before Cover” means that the premiums must be paid before the insurance policy can be issued.

What is the minimum premium required per policy?

A minimum premium of RM5,000 excluding stamp duty and service tax is required to incept a new policy.

Are the premium rates and renewal of policy guaranteed?

Premium rates and its renewability are non-guaranteed. We reserve the right to establish, at the end of any subsequent Policy Year thereafter or whenever the terms of the basic Policy/ riders are changed, new premium rates that shall be applied to the Insured Members.

For Group Term Life, Additional Critical Illness rider and Accelerated Critical Illness rider, the premium rate is determined based on the average Age of all the Insured Members in the group. Thus, the premium rate is reviewed yearly based on the average Age of the group during renewal.

For Accidental Death & Disablement and Repatriation of Mortal Remains rider, the premium rate is on per Insured Member basis.

For Group Hospitalisation & Surgical, Out-Patient Clinical rider, Dental Benefit rider, Out-Patient Mental Illness Treatment rider and Direct Paediatrician Benefit rider, the premium rate is on per Insured Member basis. It is determined based on the Policy’s claim experience, cost of medical treatment, advancement in medical technology, etc.

What happens when an employee resigns or when there are new employees?

When there are any changes affecting an insured member's coverage, please inform us through email EB.Life@allianz.com.my and include your intermediary in the email.

A pro-rated premium shall be refunded for any employee resignation and a pro-rated premium will be charged when a new employee joins the organisation after the policy commencement date.

Can I change or upgrade my employees’ benefits?

Yes, change or upgrade of benefits can be done upon the policy anniversary/renewal, subject to our acceptance. It will be applicable across the same employee category.

Switch of employee category due to promotion or demotion is applicable if you inform us within 30 days prior to the benefits eligible date. A pro-rated premium will be charged or refunded.

Does Allianz EliteChoice provide overseas coverage?

If the Insured Member chooses to or is referred to be treated outside Malaysia by the attending physician, the hospitalisation and surgical benefits payable will be based on the reasonable and customary and medically necessary charges for such an equivalent treatment in Malaysia. The transportation cost to the place of treatment is excluded.

For overseas Out-Patient General Practitioner and Specialist Care, the benefits are payable based on the reasonable and customary and medically necessary charges for such an equivalent treatment in Malaysia, within the limits as stated in the Schedule of Benefits.

What are the terms and conditions of cashless facility?

The cashless facility is provided as an option for GHS applications. If Out-Patient Clinical rider is selected, cashless facility is mandatory.

With cashless facility, the Insured Member can access panel hospitals and panel clinics without paying for admission subject to the limits specified in the Schedule of Benefits, including Non-emergency Out-Patient General Practitioner Care.

Can “take-over” of previous policy conditions from previous insurer be applied?

Yes, “take-over” condition is applicable for Allianz EliteChoice. A copy of existing insurer’s Schedule of Benefits, Member Schedule, current and last 2 years claims history are required for pre-approval take-over consideration. The period of cover must continue from the last due date of the policy from the previous insurer before any take-over can be granted. However, we reserve the right to decline any take-over application.
